We're a mission-driven team building technology and services that help communities address social drivers of health. Our founders started Activate Care in 2012 after experiencing firsthand how fragmented the healthcare system can be when you're trying to navigate care for yourself or your family. We saw that too many people were being screened for needs and then simply referred out - only to fall through the cracks up to 80% of the time.
Today, we combine our cloud-based CareLink platform with Path Assist, our tech-enabled community health worker program, to create a more proactive approach to social care. We work with health plans, healthcare systems, government agencies, and community organizations to coordinate care, track outcomes, and actually connect people to the resources they need. Our team includes public health professionals, nurses, community health workers, engineers, and data scientists - all focused on the same goal of making health equity a reality rather than just a buzzword.
