We started Trella Health in 2015 with a clear mission: to empower meaningful change in healthcare through performance visibility. Our team builds market intelligence, CRM, and EMR referral management solutions that help healthcare organizations - especially in post-acute care - understand their markets, identify growth opportunities, and build better relationships with referral partners. We’ve grown from a small startup to over 150 team members, serving more than 1,000 customers including 10 of the top 10 post-acute providers and 4 of the top 10 health systems.
What drives us is the belief that healthcare shouldn’t be a guessing game. Our platform provides actionable insights drawn from billions of performance metrics and referral data, representing 90% of all lives 65+ in the U.S. population. When hospitals, home health agencies, hospices, and skilled nursing facilities can see clearly where they stand and where they can improve, everyone benefits - especially patients. We recently merged with Repisodic to expand our capabilities in hospital discharge automation, and we’re proud to be one of only a few companies designated as both a Qualified Entity by CMS and an Innovator under its Virtual Research Data Center Program.
