Trillium Health Partners is a community-based, academically affiliated hospital network serving Mississauga, west Toronto and surrounding Ontario communities. Its technology work spans integrated clinical information systems, digital health and health IT, care coordination and patient engagement technology, and the systems underpinning home-based care delivery. Technical staff work alongside clinical and academic teams across an organisation that serves more than 1.7 million patients a year.
The network operates five sites: Credit Valley Hospital, Mississauga Hospital, Queensway Health Centre, the Reactivation Care Centre at the Church site, and the THP-UHN Reactivation Care Centre, run jointly with University Health Network. Mississauga Hospital is undergoing major infrastructure renewal through the Peter Gilgan Mississauga Hospital project. THP@home, the organisation's service delivering personalised care in patients' appropriate settings outside hospital, extends technology and care delivery beyond the traditional facility footprint.
Trillium Health Partners' Plan to 2030 sets out a model of care defined by the health and well-being of patients and communities rather than illness alone. The organisation combines patient-centred clinical care with academic training and education, with an emphasis on coordinated care, patient engagement and operational excellence.
